Tretinoin is widely considered the clinical gold standard of anti-aging and acne treatments, per Kania et al., Journal of Cosmetic Dermatology 2024 (PMID 39533523). However, its potency means that improper use can lead to irritation and the dreaded “tretinoin purge.” For a detailed comparison with a gentler active, see our guide on azelaic acid vs tretinoin or learn about the differences between tretinoin vs retinol.

The Tretinoin Purge Timeline

  • Weeks 1-2: Initial dryness and possible irritation. Start slow (2-3 nights per week).
  • Weeks 3-6: Peak purging (new breakouts as skin turnover increases).
  • Weeks 8-12: Improvement in texture and reduction in new acne.

The “Sandwich Method” for Irritation

If you have sensitive skin, try applying moisturizer before AND after tretinoin:

  1. Wash face with a gentle cleanser.
  2. Apply a thin layer of moisturizer.
  3. Wait 15 minutes.
  4. Apply a pea-sized amount of tretinoin.
  5. Apply another layer of moisturizer.
